NASH Treatment in India: Medicines, Surgery, and Liver Transplant Explained

August 04, 2026

NASH is a progressive liver condition in which excess fat triggers inflammation and damages liver cells. The condition is now called metabolic dysfunction-associated steatohepatitis, or MASH, under updated medical terminology. It develops within the broader group of metabolic fatty liver diseases and may gradually lead to fibrosis, cirrhosis or liver failure.

Effective NASH Treatment in India depends on the amount of liver damage, the presence of metabolic conditions and the patient’s overall health. Most patients need lifestyle changes and treatment for diabetes, obesity or high cholesterol. Medicines may help selected patients, while advanced cirrhosis may require liver transplantation.

What Causes NASH?

Understanding What Causes NASH helps patients address the factors driving liver inflammation. NASH usually develops when fat accumulates in the liver alongside one or more metabolic risk factors.

Common risk factors include excess body weight, abdominal obesity, insulin resistance, type 2 diabetes, high triglycerides, abnormal cholesterol and high blood pressure. Poor diet, limited physical activity, sleep-related disorders and genetic susceptibility may also contribute.

Not every person with fatty liver develops NASH. However, patients with diabetes, obesity or several metabolic conditions face a greater risk of inflammation and progressive liver scarring.

Symptoms of NASH

The early Symptoms of NASH often remain mild or completely absent. Some patients experience persistent tiredness, weakness or discomfort below the ribs on the right side.

Symptoms become more noticeable when substantial fibrosis or cirrhosis develops. These may include jaundice, abdominal swelling, swollen legs, itching, poor appetite, muscle loss, easy bruising or confusion.

Symptoms alone cannot confirm NASH. People with metabolic risk factors may need liver assessment even when they feel well.

How NASH Diagnosis Is Made

A thorough NASH diagnosis usually begins with a review of medical history, alcohol intake, medicines, weight and metabolic conditions. Blood tests help assess liver injury and rule out other causes, although normal liver enzymes do not always exclude significant disease.

Ultrasound can identify excess liver fat. FibroScan and magnetic resonance elastography measure liver stiffness and help estimate fibrosis. Doctors may also calculate non-invasive fibrosis scores using age and routine blood-test results.

A liver biopsy is not necessary for every patient. A specialist may recommend one when non-invasive tests provide unclear results or when confirming the disease stage would affect treatment.

Lifestyle-Based Fatty Liver Treatment

Lifestyle care forms the foundation of Fatty liver treatment and remains important even when medicines are prescribed. Gradual and sustainable weight reduction can lower liver fat and inflammation. Greater weight loss may improve fibrosis in some patients, but crash diets should be avoided.

A balanced diet should focus on vegetables, fruits, whole grains, pulses, lean proteins, nuts and unsaturated fats. Sugary drinks, refined carbohydrates, highly processed foods and excess saturated fat should be limited.

Regular physical activity can improve insulin sensitivity, cardiovascular health and weight control. The NHS recommends aiming for at least 150 minutes of exercise each week, adjusted according to the patient’s fitness and medical condition.

Medicines Used in NASH Treatment in India

There is no single medicine suitable for every patient with NASH. Treatment plans often include medicines that manage the metabolic conditions contributing to liver damage.

Doctors may prescribe appropriate treatment for diabetes, obesity, high cholesterol or hypertension. Some diabetes and weight-management medicines may also improve metabolic health and reduce liver fat, although suitability varies between patients.

Newer medicines designed for selected adults with MASH and moderate-to-advanced fibrosis have entered international clinical practice. However, approvals, availability and prescribing criteria differ between countries. Patients should consult a qualified Liver Specialist in India before considering any NASH-specific medicine.

Vitamin E or medicines such as pioglitazone may be considered in selected cases under specialist supervision. They do not suit every patient and may carry risks. Herbal remedies and unregulated liver supplements should also be avoided unless a doctor confirms their safety.

Is Surgery Used to Treat NASH?

Doctors do not usually perform liver surgery to remove NASH because the disease affects the whole liver rather than one isolated area.

Metabolic or bariatric surgery may help carefully selected patients with obesity achieve significant, lasting weight loss. This can improve metabolic health and may reduce liver inflammation. However, it is not suitable for everyone, especially patients with advanced or decompensated cirrhosis.

A multidisciplinary team should assess the benefits and risks before recommending weight-loss surgery. Surgery must also be followed by long-term nutritional support, physical activity and medical monitoring.

When Is a Liver Transplant for NASH Needed?

A Liver transplant for NASH becomes relevant when the disease progresses to advanced cirrhosis, liver failure or certain cases of liver cancer. Transplantation is not required for simple fatty liver or early fibrosis.

Warning signs of advanced disease may include repeated abdominal fluid accumulation, internal bleeding, confusion, severe jaundice, infections or worsening kidney function. A transplant team evaluates liver function, heart health, diabetes control, body weight and the patient’s ability to manage lifelong follow-up.

During transplantation, surgeons replace the damaged liver with a healthy liver from a deceased donor or part of a liver from a living donor. Liver transplantation may become the only effective option when advanced cirrhosis causes irreversible liver failure.

Recovery and Long-Term Monitoring

Recovery depends on the disease stage and the treatment provided. Patients with early NASH may see improvement through weight management, exercise and better control of metabolic conditions.

Regular monitoring may include blood tests, fibrosis scores, imaging and liver-stiffness assessments. Patients with cirrhosis need closer follow-up to identify complications and screen for liver cancer.

After transplantation, patients require lifelong anti-rejection medicines, infection prevention, metabolic risk management and regular transplant reviews. Healthy weight control remains important because fatty liver disease can develop in the transplanted liver.

Consulting Dr Prashant Kadam

Patients with advanced fibrosis, cirrhosis or suspected liver failure should consult an experienced liver specialist or transplant team. Dr Prashant Kadam is an internationally trained liver transplant and HPB surgeon who provides evaluation and surgical guidance for complex liver conditions.

Appropriate NASH Treatment in India should combine accurate diagnosis, personalised metabolic care and regular monitoring. Early treatment can often slow disease progression, while timely transplant assessment may provide an option for patients who develop irreversible liver failure.
